Financial and Cancellation Policies:

• All charges are to be paid in full at the time services are rendered.
• Accepted payment methods are: cash, debit cards, credit cards (Visa, MasterCard, Discover, or American Express), Care Credit, Scratchpay, or checks.
• The account holder must be present and have a valid photo ID when paying with Care Credit, Scratchpay, or a check.
• Checks will not be held or post dated.
• If a check is returned for any reason, a fee of $40 will be due immediately and the privilege of writing a check will be revoked.
• A $65 deposit is required when scheduling any anesthetic procedure. At least 48 hours notice of cancellation is required, or the deposit will be forfeited.
• For any new client (not seen by the doctor within the past year): A deposit, equal to the cost of the type of office visit being scheduled, is required when scheduling an office visit. At least 24 hours notice of cancellation is required, or the deposit will be forfeited.
• A deposit may also be required prior to initiating diagnostics and treatment.
• For any existing clients: At least 24 hours notice of cancellation is required for all office visits or the visit will be recorded as a "No Call No Show." After five (5) recorded "No Call No Show" appointments, a deposit, equal to the cost of the type of office visit being scheduled, will be required when scheduling an office visit. At least 24 hours notice of cancellation is required, or the deposit will be forfeited.
